In order to break the conflict between the proposed heading "Levin, Yisra'el" and the identical 400 on the record for "Levin, Israel", one should add a date qualifier to the latter. The date needs to be added only to the otherwise conflicting ref, not to the heading and other refs in that record. Breaking a conflict with a date is preferable to breaking it with any other type of qualifier, including $c-types like "'o. d.," though these have their place if dates are not available.
In another case, it turned out that "Ish Shalom, Mordekhai" normalizes
to the same thing as another heading, so the qualifier
"rav di-k.k. Teth" was added to that ref only.
While the order of qualifiers is supplied in Chapter 22, the instruction to add distinguishing info to the ref. is found in RI26.2C "See also" references. LCRI 22.17-22.20, p. 1 gives the order of preference for breaking conflicts.
